The City of Elmhurst and the Village of Berkeley are partnering with Elmhurst Christian Reformed Church and Jewel-Osco to host a vaccination event for all members of the public age 16 and older. The event will take place on Tuesday, May 4 from 9 a.m. to 5 p.m. at Elmhurst Christian Reformed Church (149 West Brush Hill Road, Elmhurst).
Included in this message is a link for you to register for the vaccine. 1,600 of the two-dose Pfizer COVID-19 vaccine are being made available to the public by appointment only. The second dose will be administered on Tuesday, May 25, at the same location and same time as the first dose appointment time given. Only those on the vaccination list will be approved to receive the vaccine. No walk-ups or transfer of names allowed.
Medical staff from Jewel-Osco will be administering the vaccine. Please bring your state I.D. and a photocopy of your health insurance or Medicare card (if applicable). You are able to receive the vaccine regardless of insurance coverage or immigration status. Parking will be available on site. Please follow the direction of staff upon arrival.
