[ARCHIVED] City of Elmhurst: Vaccine Event 4/8/21

Elmhurst Resident,

The City of Elmhurst is partnering with Jewel-Osco and Elmhurst Christian Reformed Church to host a vaccination event for Elmhurst Residents who fall into the vaccine recipient phase category 1a, 1b, 1b+, or 1c. The event will take place on Thursday, April 8 from 10am to 6pm at Elmhurst Christian Reformed Church (149 W Brush Hill Rd, Elmhurst). The second dose will be administered on Thursday, April 29th at the same location. 

Included in this message is a link for you to register to receive both vaccine doses. 1,000 two-dose Pfizer vaccines are being made available to Elmhurst Residents by appointment only. Only those on the vaccination list will be approved to receive the vaccine. No walk-ups or transfer of names allowed.

The City of Elmhurst and Jewel-Osco will not be collecting any billing or payment information, and medical staff from Jewel-Osco will be administering the vaccine. Please bring a copy of your state I.D. If you have health insurance or medicare, please bring verification. Parking will be available on site. Please follow the direction of staff upon arrival.
